If you are interested in enrolling your child(ren) in the Boys & Girls Club of Metro Queens After-School Program for the 2026–2027 school year, we encourage families who currently receive Cash Assistance or meet the attached income eligibility guidelines to complete the ACS (Administration for Children's Services) Child Care Voucher Application.
Please Note: Families who complete the ACS MyCity application and qualify for child care assistance receive higher priority for enrollment. We strongly encourage all eligible families to complete the application as soon as possible.
Step 1: Complete the ACS MyCity Child Care Voucher Application

Sign in with your NYC.ID account or create one if you do not already have an account.
Select the Correct Application
  • Under "Select a Type of Assistance," choose "New Child Care Voucher."
  • Click "Select Child Care Voucher," then click "Continue."

Please enter the following information:

Required Documents
Please upload:
  • Parent's valid State-issued photo ID
  • Child's birth certificate
  • Current pay stubs
Pay Stub Requirements
Weekly Pay
  • 4 current, consecutive pay stubs if your gross pay is the same each week.
  • 12 current, consecutive pay stubs if your gross pay varies.
Bi-Weekly or Semi-Monthly Pay
  • 2 current, consecutive pay stubs if your gross pay is the same.
  • 6 current, consecutive pay stubs if your gross pay varies.
If You Are Currently Unemployed
If you are actively seeking employment, you may upload a Work Search Record instead of pay stubs.

After submitting your ACS MyCity application, please email us your MyCity login information (username/email and password) so we can monitor your application and assist if ACS requests additional documentation.
Step 2: Complete the MyClubHub Application
To enroll your child(ren), the MyClubHub application must also be completed.

Steps to Complete the MyClubHub Application
  1. Log into your MyClubHub account.
  2. Click "Browse Memberships."
  3. Select PS 97Q and click "Save Club."
  4. Click "CLICK TO SEE MORE!"
  5. Select "97: WAITLIST SY '26–'27" and click "Select."
  6. Choose your child(ren) and click "Next."
  7. Complete all required fields marked with a red asterisk (*).
  8. Click "Next" and sign all four forms by clicking the blue "Sign" button.
  9. Click "Finish" in the top-right corner, then click "Finish" again on the confirmation page.
  10. Once submitted, you should see a "Transaction Complete" screen.
